Because minors are involved in the Visible Men movement, we are obligated to take extra precautions about who we feature as role models. You agree and understand that if any of the information given by you during the application process is found to be false, misleading, incomplete, or inaccurate, or if it is found that you have omitted from this application any material information, your application may be rejected for that reason. Further, you agree and understand that, if you become a Visible Man, and subsequently the information given by you during the application process is found to be false, misleading, incomplete or inaccurate, or if it is found that you have omitted from this application any material information, you will be subject to immediate termination as a Visible Man, without notice at any time.

In making this application to become a Visible Man, you understand that Visible Men routinely obtains background reports and/or other information regarding volunteer applicants from Backgrounds Online. In connection therewith, Backgrounds Online ,a consumer reporting agency, may provide a report to Visible Men which may include information as to your criminal record, if any, driving record, character, general reputation, personal characteristics, mode of living, and other information. You request, authorize and consent to the release and disclosure of any and all information including but not limited to the above to Visible Men and any such consumer reporting agency. You understand that acceptance as a Visible Man is conditional on my successful completion of a background check.

Law enforcement agencies and other entities for positive identification purposes require the following information when checking public records. It is confidential and will not be used for any other purposes. I hereby release Visible Men and its agents, and all persons, agencies, and entities providing information or reports about me from any and all liability arising out of the request for or release of any of the above mentioned information or reports.
